  • Read ALL the information about “breast screening” and “program guidelines”
  • Fill out the “eligibility questionnaire” (hint: check “yes” on the provider question). Submit
  • Donate Box $5 (or whatever you choose)
  • Click “Donate”

You will receive an email within a week indicating the amount of assistance you will be awarded.

Example:

“UBCF has awarded you a grant that will pay up to $150.00 for your breast screening.”

Important: Read the email in its entirety and follow the rest of the instructions. (You need to respond.)
